This Agreement, as amended from time to time, governs your use of MIC’s Provider Portal and applies when you access this service to review patient reports and appointment details.
- By signing this agreement, you acknowledge that you have read and understood all requirements for the use of this service. Failure by you or your affiliates to comply with these requirements may result in the suspension or termination of this service. You further agree that you or your affiliates have read and fully understand the Alberta Health Information Act (HIA), the Personal Information Protection and Electronic Documents Act (PIPEDA), and the requirements for the safety and confidentiality of identifying health information.
- Use of Information: Use of information accessed via the MIC Provider Portal is intended to support continuing patient care, and only information relevant to this purpose is to be accessed.
- Access to Reports: You understand and agree that access to information only applies while at the facility(s) indicated within this agreement. You may view reports but may not copy or store them unless using an industry standard and in compliance with the Alberta Health Information Act and the Personal Information Protection and Electronic Documents Act.
- Monitoring and Audits: MIC will conduct periodic monitoring and audits for appropriate searches.
- Passwords and Multi-Factor Authentication (MFA): The first time you log in, you will be prompted to change your password (it must be 12-16 characters) and set up Multi-Factor Authentication. After five failed login attempts, you will be locked out of the account and will need to contact MIC to reset your password either by phone or email.
- Security Expectations: Once granted access to the Provider Portal, it is the users’ responsibility to protect the privacy of their account. Passwords and account access for MIC’s Provider Portal are granted on an individual basis and are not to be shared with other employees or coworkers under any circumstances. Each time you verify your identity through MFA, you will be granted a 4-hour window to work in the environment.
Notification of Changes: It is the MIC Provider Portal physician custodian’s responsibility to notify MIC Medical Imaging of any change of status of individuals listed within this agreement.
